The issue

High-quality healthcare begins long before hospital admission and continues well after discharge. Outpatient and preventive services are the first point of contact for most Nigerian patients and represent the most cost-effective opportunity to detect disease early, manage chronic conditions, and reduce avoidable hospitalizations and emergency presentations. Standards should address outpatient clinic organization and appointment systems, waiting time reduction, chronic disease management for conditions such as hypertension, diabetes, sickle cell disease, and HIV, immunization services, cancer and communicable disease screening, antenatal and well-child care, health education and counseling, medication refill and adherence support, post-discharge follow-up, and structured referral pathways linking primary, secondary, and tertiary levels of care. Facilities should also adopt the concept of a medical home, a designated primary care base that coordinates the management of chronic illness and supports long-term patient wellbeing.
